Hola, como se llama usted? (As opposed to "Como te llamas?" which is informal).

Asking about medications and prior surgery is in an older entry, so is how to tell someone you've made an appointment for them to see a specialist. It's impolite to say "Tendremos que mandar te a ver un especialista" as it translates to "We'll have to send you to see a specialist." In Spanish, it's impersonal and just earns you negative points.
Also, if you're going to leave the room, here are a few useful phrases so your patient doesn't feel left hanging:
I will be back in a few minutes.
Regresaré en unos minutos.
I will be back with your prescription.
Regresaré pronto con su prescripcion.
Thank you for coming, take care.
Gracias por venir, cuidase bien.
